In order to recieve the correct accommodations for your disability, you should meet the following criteria.
Criteria
Documentation should be conducted by a qualified professional and include the evaluator's name, title, credentials, license number, signature and date of evaluation, all on letterhead stationery.
Documentation should include a diagnostic statement identifying the disability and date of the original diagnosis.
Documentation should include a description of the diagnostic criteria and/or diagnostic test(s) used.
Documentation should include a description of the current functional limitations of the disability in an academic environment.
Documentation should include medication (dosage and existing side effects), assistive devices/services, treatments currently prescribed or in use.
Documentation should include a description of the expected progression or stability of the impact of the disability over time.
Documentation should include recommendations (OR STRONGER LANGUAGE) for accommodations, adaptive devices, assistive services, and compensatory strategies to compensate for the functional limitations.
You certainly need to know the functional impact of each separate condition.
